Instructions for completing the
Agenda Items Submission Form

The Agenda Items Submission Form provides clarification about the proposal, and any related concerns or additional information that surround the issue that may assist the Council during their deliberations on the issue. Without sufficient information Council members may not address your concerns adequately or they may focus on another aspect of the issue. Your statement of desired outcomes serves as a guideline for Council members, not a guarantee of desired outcome. It is your responsibility to insure that your College Representative to the Council is fully informed of your proposal. Ideally, they will work with you in preparing all materials associated with your proposal. It is also your responsibility to fully explain the justification for any changes in policy that are proposed. In most cases, this will form the basis upon which the Council will make their decisions.

Contact Information: Please provide contact information so we may seek additional clarification, if necessary, and so we can keep you informed on the status of your issue. An email address is requested to expedite communication with you.

Agenda Topic: When appropriate, please use the current name of the policy to be reviewed, or if requesting creation of a new policy, please suggest a name that is descriptive of the policy content, e.g., clarification of existing policy: Course Reuse: a course applying to more than one degree requirement.

Description of Topic:  Give a brief overview of current policy or new policy to be reviewed.

Issues Involved:  Give a description of all issues, problems, concerns with current policies and procedures as applicable. Provide all background information that you believe will be helpful as the Council considers this issue. This is generally where justification for policy change will be explained. Please recognize that you may be asked by your Council representative to provide additional background information, as the Council deems necessary.

Expected Outcomes:  Explain the outcome(s) you feel is/are desirable and state why. This may involve outcomes in programs or areas of the University other than are directly affected by the proposal. Please attempt to anticipate these and include them in your outcomes analysis.
